Small water systems, especially those in rural areas, are much more likely to fail water quality standards. This is a big deal for people&#8217;s health. Our infrastructure is old, source water is often dirty, and local budgets are tight. After the Flint lead crisis, we really needed to check on U.S. drinking water quality. But nobody had done a nationwide study on water quality violations over many decades. We need to reduce these violations because in 2015, almost 21 million people relied on water systems that didn't meet health standards. This paper looks at where and when health-related Safe Drinking Water Act violations happened. We used data from 17,900 community water systems between 1982 and 2015. We also pinpointed factors that make communities and water systems vulnerable. We found more violations over time and \"hot spots\" in several states, especially in the Southwest. Repeat violations are common in these hot spots, meaning those water systems just can't seem to get it right. When it comes to vulnerability, rural areas have many more violations than cities. Private ownership and buying water from another source seem to help with compliance. These findings show which systems need help to consistently meet standards.
